Competitive Program Player Placement Request Form

This form is used for interested parents to request that their player be evaluated for an older age group. If you have more than one player you are making a request for, you must fill out a new form for each player. It is important to note that if players are approved to attend placements in an age group older than their own, the parent will receive an email approval from CUSC Sporting Director, Brendan Cook. Parents/players must have this email approval in order to attend placements outside of their assigned age group. Additionally, all players approved to attend an older placement must also attend their true age group placements, totaling 4 sessions in which they will be evaluated.
